What Your Day Will Look Like

As a Provider Onboarding & Credentialing Associate, the daily tasks include coordinating provider onboarding processes, maintaining compliance documentation, and managing credentialing applications. This role ensures providers are activated and ready for service, supporting Hazel Health’s mission to deliver quality care.

This full-time contract role is remote, providing flexibility for professionals across the United States. Note: This role is not eligible for applicants residing in AK, ID, KS, LA, NH, ND, OK, SD, WV, or WY.

Responsibilities & Expectations

  • Coordinate Onboarding: Manage provider setup processes
  • Track Documentation: Ensure compliance and timelines
  • Maintain Records: Update data in Salesforce and CertifyOS
  • Monitor Credentialing: Oversee application completion
  • Collaborate with CVO: Ensure credential compliance
